We need information from you to decide if your situation is something we can help you with. This checklist will give us the basics. The name of your employer is required and if it is not filled in, we can read no further. We need your employer name because we are required to make sure we have no conflict of interest in taking on a case for you. Document Review & Consultation

We offer document review and a phone consultation for a flat fee of $275. Payment must be made in advance through PayPal and the documents must be scanned into PDF form. In order to complete the intake process, please fill out this form to completion, upload the document you would like reviewed, submit a consultation payment using the PayPal link in the form below, and press SUBMIT to send the form.
